FORT WALTON BEACH, Fla. – Registration opens at noon on Tuesday, March 23 for COVID-19 vaccinations on March 29, March 31, and April 2 in Okaloosa County.
Those eligible for the vaccine are:
- Long-term care facility residents and staff
- Persons 50 years of age and older (Born March 1971 or earlier for March dates or April 1971 or earlier for April dates.)
- Non-hospital health care workers with direct patient contact
- Persons 16 and older (Pfizer-BioNTech) or 18 years of age and older (Johnson & Johnson) deemed to be extremely vulnerable to COVID-19 by a physician. Must have completed COVID-19 Determination of Extreme Vulnerability Form or form with equivalent information from a physician. The DOH COVID-19 Determination of Extreme Vulnerability form or a form with equivalent information from the physician’s office must include the physician’s license number.
The vaccination provided on March 29 and March 31 will be the Pfizer-BioNTech COVID-19 vaccine. A second dose of the Pfizer COVID-19 vaccine is required 21 days after the first dose.
The vaccination provided on April 2 will be the Johnson & Johnson vaccine. The Johnson & Johnson vaccine requires one dose.
For appointments not filled through the pre-registration system, registration will open at noon central time on Tuesday, March 23 both online and for scheduling over the phone. Appointments cannot be made through email or by walk-in at the health department.

Individuals receiving the vaccine must complete a vaccine screening and consent form. For those that cannot access the forms in advance, a paper copy will be provided at your appointment time. A screening and consent form is required for each dose of the vaccine.
